I am writing to inform you that Casalim Family Medicine will be closing on 12/31/2026. Until then, I will be available to see you for ongoing care.

Please contact the office for an appointment and/or copies of your medical records. If you are so inclined, we can also send your records to a new primary care provider.

It has been truly an honor for me to be part of your healthcare journey, and I wish you continued good health and fortune.
